The obverse features Ontario's Coat of Arms and the inscription [ONTARIO]. The reverse features the province's floral emblem, the Trillium.
Additional periods of 5 year service are denoted by a Trillium Device as follows:
25 Years of Service - 1 Silver Trillium device
30 Years of Service - 2 Silver Trillium devices
35 Years of Service - 1 Gilt Trillium Device

Recipients also receive the Police Exemplary Service Medal from the Governor-General for the same 20 years of service.
The Medal was granted status in the Official Precedence of Canadian Orders, Decorations, and Medals on April 2, 1998.
