The Loyal and Faithful Service Medal was instituted by the ruler of Dewa Junior Branch State, His Highness Malhar Rao Baba Sahib Puar, to recognize faithful and loyal service.

The reverse is thought to feature an engraving of the recipient's name or other details in Persian script.
