The ribbon of the Medal was the inverted pattern of the Cross ribbon, although this is not mentioned in any official documents. There are many ribbon patterns (size, colour, stripes) and there is conflicting information about them - both in primary sources and in secondary sources.

The Decoration for the Anniversary of the Inauguration of Leopold I was established by Royal Decree on July 20, 1856.

The Medal was awarded to Non-Commissioned Officers and other ranks of the Belgian Armed Forces or the Civil Guard for 25 years of uninterrupted service as of July 21, 1856.
