This medal was instituted on December 28, 1918. It's original name is Medaglia ai volontari nell'arma commbattente/nell'ospedale da guerra, and was awarded to individuals who volunteered in the army or as medical personnel. The medal awarded to army volunteers was issued in gold, silver and bronze. The medal awarded to medical volunteers was issued in silver and bronze.

Although San Marino was geographically very close to the front, the Republic never entered the First World War. However, two groups of 10 individuals did volunteer along side the Italians. The first group were combattantes and the second group as medical corp operating a Red Cross field hospital near Trieste.
