The Indian Title Badge was instituted by King George V on the occasion of the Delhi Durbar on December 12, 1911. The Medal was awarded in 3 classes to civilians and the Viceroy's Commissioned Officers of the Indian army in recognition of faithful service and acts of public welfare.
There are multiple versions of the Medal which vary by the obverse design of the ruling monarch. The first 2 versions of the Medal were issued during the reign of King George V. The first version features a right facing depiction of King George V's bust while the second version features a left facing depiction of King George V's bust. A third version of the Medal features a left facing bust of King George VI.
The Medal also differs by the obverse inscription of the title of the recipient. Hindu recipients of the III Class Medal receive a Medal which features the inscription "RAO SAHIB" or "RAI SAHIB" while Medals inscribed "KHAN SAHIB" are awarded to Muslim recipients.
