There is no information on the institution of this award. However, the earliest it could have been instituted was May of 1943, and the latest it was awarded was April of 1944. It was awarded with an accompanying award document.
The award is rectangular with a raised edge. It shows a national eagle with folded wings and swastika. The eagle stands on the crests of the cities of Nürnberg (Nuremberg) and Wiesbaden. The inscription reads: for special merits / the commanding general and chief of Air District XII.
The reverse is semi-hollow.
Some sources claim that there is a bronze and a silver version, however, this is false.
The German title of this award is "Eiserne Ehrenplakette des Luftgaues XII".
