The medal was instituted by Peter the Great. It was conferred in commemoration of the taking and storming of the Noteburg Fortress during the Northern War. The medal was awarded to the members of the Preobrazhensky and Semjonovsky.

The obverse features an inscription, translated as "Tsar Peter Alekseevich Sovereign of Russia." The reverse features an inscription, translated as "Taken 21 October 1702" (the date should read October 12, 1702, as it was the date of the storming of the fortress), and "Held by the Enemy 90 Years."
