The Royal Order of San Carlos Borromeo (Real Orden de San Carlos Borromeo ) was probably instituted by Carlos V, the first of the Carlist claimants to the throne of Spain, during the First Carlist War (1833-1840). However, the Order was approved and regulated by Carlos VIII, inheritor of Carlos V, on March 29, 1947.

The Order was awarded to Spanish and foreign citizens, or military personnel, in recognition of the loyalty and fidelity shown to the Carlist cause. It was mainly awarded to the veterans of the Carlist Wars, and their heritage.

The Order is currently active in Spain, and it is awarded exclusively to the inheritors of the Carlist branch. It is awarded to women in a reduced size, and a bow ribbon.

The grades of the Order are Grand Collar, Grand Cross, Commander by Number, Commander, Knight, Silver Medal.

The Order has no motto.

The Collar has 39 links alternating the Saint Andrew shield and Flour the Lys,
